Before your CBD evaluation appointment with a CBD-Doctors licensed physician, it’s imperative that you obtain a copy of your past medical records. In order to get cannabidiol treatment in any state, you must first obtain a copy of your medical records from your primary care physician.

If you arrive to your appointment without these records, your CBD-Doctors licensed physician will likely be able to request the records on your behalf, but this will delay the process of obtaining the medical CBD recommendation needed.


Some practices may require you to fill out a specific form to request your medical records. To obtain this form, simply call your primary care physician’s office and request a copy. They should be able to deliver it to you by fax, email, postal mail or you may pick it up from the physician’s office.

If your physician’s office doesn’t have a specific form, simply write a letter to request your records and be sure to include:

  • Your name, including your maiden name (if applicable)
  • Social security number
  • Date of birth
  • Address and phone number
  • Email address
  • Record(s) being requested
  • Dates you were under your primary physician’s care
  • Signature
  • Delivery option (pick up, fax, email, etc.)

If you do not have a primary care physician, are uninsured, have not had a check-up in a while, or cannot afford to see a physician, your CBD-Doctors licensed physician may be able to assist in finding an affordable clinic in your area that can give you a proper medical exam and diagnosis.
